Start by scoping honestly. Most specialized builds arrive as a broad ambition. A short discovery sprint turns it into a defined first release with a cost attached, and occasionally concludes that the thing should not be built at all, or should be bought. We would rather deliver that conclusion in week three than in month nine.

Ship something real early. A working slice in production beats a comprehensive specification. For founders this matters commercially — a usable MVP raises money and attracts customers in a way that a deck does not.

Assume compliance from line one. Anything touching patient data needs HIPAA-grade handling, audit logging and access control built in structurally. Retrofitting compliance is dramatically more expensive than designing for it, and for AI and teledentistry it also constrains what the product may legally do — which is worth establishing before, not after, the build.

Plan for who runs it. The question we ask early is who maintains this in eighteen months. If the answer is nobody, that shapes the architecture.
