How we approached it

Building a front door patients come back to

The client had a portal already and almost nobody used it. That is the usual situation, and the usual diagnosis is wrong — the problem is rarely the interface. It was that the portal showed an appointment date and a balance the patient already knew about, behind a password they had to remember.

We rebuilt it around reasons to return. Treatment plans with real numbers, showing the insurance estimate and the patient's out-of-pocket separately, so a proposed plan could be reviewed at home with a partner rather than decided in the chair. Balances payable in a tap. Intake and medical history completed before the visit and flowing into the practice-management system. Family scheduling, because dentistry is a household purchase and a parent managing three children's recall is a weekly user rather than an annual one.

Authentication was where we spent disproportionate effort. Most portal abandonment happens at the login screen, so we moved to magic links and SMS verification rather than passwords, while keeping the HIPAA requirements — encryption, role-based access, full audit logging — intact.

Scheduling was integrated properly rather than as a request form. Availability came from the practice-management system with provider, operatory, appointment type and duration rules applied, and confirmed bookings wrote straight back, so patients could only take slots that genuinely existed.

Adoption followed usefulness, as it generally does. Online booking and payments were used immediately because they removed obvious friction; portal return visits rose once treatment plans and family scheduling gave people a reason to log in more than once.
